Job Description:

We are seeking a dedicated Home Support Worker to join our team. In this role, you will provide a range of supports to clients in their own homes, including domestic assistance, meal preparation, unaccompanied shopping, personal care and respite care.

As a Home Support Worker, you will work directly with clients, building strong relationships and providing tailored support to meet their individual needs. You will also be responsible for maintaining accurate records and reporting any changes or concerns to the relevant authorities.

Responsibilities:
  • Provide high-quality care and support to clients in their own homes
  • Conduct regular assessments to identify client needs and develop personalized care plans
  • Deliver domestic assistance, meal preparation, unaccompanied shopping, personal care and respite care as required
  • Maintain accurate records and report any changes or concerns to the relevant authorities
Required Skills and Qualifications:

To be successful in this role, you will need to have:

  • Certificate 3 in Individual Support/Aged Care/Disability/Home and Community Support (or equivalent qualification)
  • Current First Aid and CPR certificate (must be kept up to date throughout your employment)
  • Valid Working with Children Check
  • Ability to travel within our service delivery area
  • Good level of physical fitness to meet the demands of the role
  • Current Victorian Driver's Licence
  • Ownership of a registered and comprehensively insured vehicle
  • Smartphone for work-related communication
