About the Role

This is an exciting opportunity for a motivated and experienced professional to join our team as a Support Coordinator.

The successful candidate will be responsible for providing high-quality support coordination services to our participants, including developing and implementing individualised support plans, coordinating community connections, and promoting independence.

To be successful in this role, you will require a strong understanding of the NDIS and its operational guidelines, as well as excellent communication and interpersonal skills.

  • Key Responsibilities:
  • Develop and implement individualised support plans that reflect the unique needs and goals of each participant.
  • Collaborate with participants, families, and other stakeholders to ensure seamless service delivery.
  • Coordinate community connections and promote social inclusion for participants.
  • Monitor and evaluate the effectiveness of support plans and make adjustments as needed.
  • Stay up-to-date with industry developments and best practices.
