We are seeking an Enrolled Nurse to join our dynamic multidisciplinary team at Hampstead Centre, a new centre of excellence in the care of older persons. As a key member of our CO-ACT team, you will play a crucial role in shaping and implementing an innovative model of care that will set new standards in patient outcomes.
Key Responsibilities- Provide high-quality patient care to older adults transitioning from acute to ongoing care.
- Collaborate with a multidisciplinary team to develop and implement care plans that meet the unique needs of each patient.
- Contribute to the development of a supportive, family-like environment where collaboration is key and input on improving systems and practices is highly valued.
- Participate in ongoing professional development to enhance your skills and knowledge in caring for older adults.
