Dedicated Community Healthcare Professional

We are seeking a dedicated and compassionate Registered Community Nurse to join our team. As a key member of our healthcare team, you will have the opportunity to make a meaningful difference in people's lives by providing high-quality care and improving their quality of life.

Key Responsibilities- Provide wound, catheter, stoma, and bowel care to clients in their homes
- Administer medications and injections as prescribed by healthcare professionals
- Manage chronic diseases and conditions in clients' homes, utilizing clinical assessment, decision-making, and problem-solving skills to deliver high-quality care
- Communicate effectively with clients, families, other clinicians, and the healthcare team to ensure seamless care coordination
- Degree or postgraduate qualification in Nursing
- Minimum 2 years of experience in general nursing, preferably in community care settings
- Current Registration with the AHPRA, with zero notations
- Genuine interest in community care and aged populations, with a strong desire to make a positive impact on people's lives
- Compassion, enthusiasm, and a friendly approach to interacting with clients and colleagues
- Strong experience in wound and catheter care, with a focus on delivering high-quality outcomes
- Advanced clinical assessment skills for chronic disease management, with the ability to think critically and make informed decisions
- Excellent verbal and written communication skills, with the ability to communicate effectively with diverse stakeholders
