Job Description
We are seeking a dedicated and compassionate Community Mental Health Practitioner to join our team. The ideal candidate will have the ability to deliver person-centered supports to young people facing mental health challenges, helping them achieve their life goals.
As a Community Mental Health Practitioner, you will play a vital role in supporting young people aged 16–21 experiencing mental health challenges. You will work closely with young people, families, and carers to provide personalized recovery-focused support through one-on-one sessions, shared supports, workshops, and group facilitation.
The successful applicant will be responsible for delivering flexible care, including nights, weekends, and sleepovers, to provide consistent and responsive support. They will also use evidence-based approaches, such as Motivational Interviewing, Brief Intervention, trauma-informed care, conflict resolution, and family-inclusive practice.
Key Responsibilities
- Support young people to return to their community, live independently, and enjoy a meaningful life through a staged approach to recovery.
- Use evidence-based approaches to deliver personalized recovery-focused support.
- Plan and deliver group programs to build skills and focus on recovery.
- Engage families and carers through education, referrals, and support.
- Collaborate with a range of services and the community to ensure a coordinated, integrated response to people's recovery goals.
Required Skills and Qualifications
The successful applicant will possess:
- Tertiary qualifications (minimum Certificate IV) in Youth Work, Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
- Current First Aid and CPR certificates.
- Experience supporting young people with complex mental health needs.
- Proven capability to implement therapeutic, recovery-oriented frameworks and practices.
