Healthcare Social Worker Specialist

We are seeking a skilled social work professional to join our multidisciplinary healthcare team in Lismore. The role involves providing comprehensive social work assessment and intervention services to patients/clients of the Northern NSW Local Health District (NNSWLHD).
This position requires working closely with diverse individuals and families, promoting health, well-being, and social inclusion. You will have the opportunity to collaborate with a range of healthcare professionals, developing and implementing individualized care plans.
About the Role- Provide comprehensive social work assessment and intervention services to patients/clients
- Develop and implement individualized care plans in collaboration with healthcare teams
- Work closely with patients/families to promote health, well-being, and social inclusion
- Participate in multidisciplinary meetings to share knowledge and expertise
