Job Title: Case Manager

We are seeking a proactive, highly organised part-time professional with initiative and drive to succeed in this role.

This position requires strong case management skills, knowledge of Home Care Packaged program guidelines and Aged Care legislative requirements.

  • Records management, including continuous quality management and improvement
  • Negotiating individual consumer budgets and coordinating complex care services to meet consumer care needs
  • Liaising with clients and carers
  • Coordinating community-based needs
  • Negotiating & marketing available services to consumers and carers

You will have independence and flexibility of hours, supporting our sales processes, as well as liaising with clients.

To successfully fill this role, you will have:

  • AHPRA Registration is highly regarded
  • Excellent verbal and written communication skills
