We are seeking a skilled Team Leader to support older people in living independently and staying connected within their community.

  • Key Responsibilities:
  • Develop and implement strategies for delivering services according to contractual agreements.
  • Ensure compliance with aged care quality standards by establishing efficient processes and procedures.
  • Promote Choices at Home to enhance awareness and engagement among seniors.
  • Foster partnerships with local NGOs, health professionals, and community groups.
  • Coordinate team processes for client service agreements, onboarding, assessments, referrals, and addressing service gaps.
  • Recruit and coordinate service volunteers.

To be successful in this role, you'll need:

  • Diploma of Community Services or equivalent qualification.
  • Proven experience in team leadership.
  • Strong background in aged care service delivery.
  • Excellent organisational and administrative skills.
  • Current Driver's Licence and First Aid Certificate.
  • Ability to obtain a satisfactory criminal records check.
