Home Care Specialist Opportunity

We are seeking a skilled Home Care Specialist to join our team in Mandurah, with travel to Bunbury as required.
This role offers an exciting opportunity to make a real difference in the lives of older Australians by providing comprehensive client assessments and developing personalized care plans. As a valued member of our clinical care team, you will oversee government-funded Home Care Packages, combining your clinical skills with care planning and coordination.
Key Responsibilities:
- Conduct thorough client evaluations and create tailored care plans to meet their unique needs
- Provide advanced holistic care services, including wound management and clinical assessments, within a case management framework
- Liaise with General Practitioners, hospitals, and allied health professionals to ensure seamless care delivery
- Participate in quality improvement processes, including case review and team meetings
- Collaborate with the community to provide clients with exceptional support
About You:
- Bachelor’s degree in nursing or relevant field
- Current Registration with AHPRA
- Up-to-date Flu vaccination (or willingness to obtain)
- Evidence of COVID-19 vaccinations
- Australian work Rights
- Police Check (valid within the last 3 years)
- Current Australian driver’s licence and ability to travel across the region as required
- Current First Aid & CPR certificate
