Comprehensive Care Nurse

Seeking a dedicated nursing professional to join our team in providing high-quality patient care and contributing to the achievement of improved health outcomes for Aboriginal and Torres Strait Islander people.
This role requires an individual with excellent clinical skills, strong communication abilities, and a commitment to working within a comprehensive model of chronic disease care.
The ideal candidate will possess a bachelor's degree in nursing, full AHPRA registration, and previous experience in a primary health setting. They should also be able to demonstrate ability to meet targets and performance outcomes, as well as support a continuous improvement model for achieving outcomes.
Key Responsibilities- Provide quality nursing practice within an Aboriginal primary healthcare model of service.
- Contribute to the medical/primary healthcare team and effectively coordinate own and team's workload to achieve results.
- Understand the principles of primary healthcare and work collaboratively with a wide variety of professionals and the public.
- Demonstrate clinic experience and implement and support a comprehensive model of chronic disease care.
- Operate within clinical policy and procedures at all times.

Requirements- Bachelor's degree in nursing or higher.
- Full un-restricted AHPRA registration.
- National Clearance Police Check.
- Previous demonstrable experience in relevant nursing field.

- Decisive listening, influencing and persuasive skills.
