Homes First Housing Specialist

The primary goal of the Homes First Case Manager is to provide flexible, trauma-informed, and culturally safe case management support to individuals and families in need. This involves thorough needs assessments, effective support planning, and connecting clients with the necessary resources.
This role requires a deep understanding of Housing First principles and the ability to work collaboratively with clients, their families, and the multidisciplinary team to achieve long-term housing goals. The Case Manager will also provide practical support and connect clients with additional services as needed.
Key responsibilities include:
- Conducting thorough needs assessments and developing effective support plans
- Providing trauma-informed and culturally safe case management support
- Connecting clients with the Homes First multidisciplinary team and other relevant services
- Collaborating with clients and their families to achieve housing goals
The ideal candidate will have a strong understanding of Housing First principles, excellent communication and interpersonal skills, and the ability to work effectively with diverse client populations.
