Home Support Service Planner

As a Home Support Service Planner, you will work collaboratively with customers to develop personalized support plans that meet their unique needs and goals. Your role will involve identifying and addressing risks that may impact the customer's ability to live independently at home.

  • Work closely with customers to design goal-oriented support plans based on recommendations from the Regional Assessment Service.
  • Provide information and coordination of services to help customers maintain independence in their homes.
  • Conduct regular reviews and develop tailored support plans to address identified clinical and care intervention needs.

Required Skills and Qualifications:
• A Certificate III in Community Services, Aged Care, or Service Coordination, or equivalent experience.
• Strong knowledge of aging issues, including dementia-related disorders.
• Proficiency in Microsoft Office, particularly intermediate to advanced skills.
• Excellent documentation and administration skills.
