We are seeking a highly skilled and compassionate professional to join our team in South Australia as a Care Manager. This is an excellent opportunity to deliver high-quality support to home care recipients and be part of a fast-growing organisation.
The successful candidate will have strong assessment and planning skills, with the ability to manage relationships and oversee service compliance for clients who choose us for Home Care Packages. You will be responsible for managing client challenges, facilitating a smooth home care experience, and supervising care delivery.
- Complete thorough assessments and develop care plans that meet individual client needs
- Mitigate client challenges and facilitate a seamless home care experience
- Supervise and coordinate care delivery to ensure high standards are met
- Review client progress notes and implement corrective actions when necessary
- Meet with prospective clients to discuss their care requirements and explain the benefits of choosing us
- Report and manage incidents and hazards in accordance with organisational policies
- Develop strong relationships with clients and their families and seek regular feedback
- Collaborate with the scheduling team to ensure staff and client expectations are met
- Chair regular team meetings to support staff in their day-to-day duties and conduct performance reviews as required
