The Social Health Care Coordinator will be responsible for delivering case management and crisis support to clients, providing a range of social and wellbeing supports to meet their holistic recovery needs.
Key Responsibilities:
- Assess and support the needs of families and individuals referred to the program.
- Provide early intervention, crisis management, and aftercare services.
- Deliver case management and crisis support to clients, working within a strengths-based framework.
- Liaise with other community organisations and government agencies to facilitate referrals and coordination of services.
Requirements:
- Tertiary qualification in Social Work or Human Services or related field.
- Demonstrated ability to apply a high level of practice judgement when working with clients.
- Ability to work with a culturally and socially diverse group of clients.
- Effective communication and collaboration skills.
- A willingness to travel across the ATSICHS Brisbane region and work across different sites.
Benefits:
- Annual salary $74,351 to $82,964 per annum (depending on experience and qualifications).
