The Social Health Care Coordinator will be responsible for delivering case management and crisis support to clients, providing a range of social and wellbeing supports to meet their holistic recovery needs.

Key Responsibilities:

  • Assess and support the needs of families and individuals referred to the program.
  • Provide early intervention, crisis management, and aftercare services.
  • Deliver case management and crisis support to clients, working within a strengths-based framework.
  • Liaise with other community organisations and government agencies to facilitate referrals and coordination of services.

Requirements:

  • Tertiary qualification in Social Work or Human Services or related field.
  • Demonstrated ability to apply a high level of practice judgement when working with clients.
  • Ability to work with a culturally and socially diverse group of clients.
  • Effective communication and collaboration skills.
  • A willingness to travel across the ATSICHS Brisbane region and work across different sites.
