About the Role

This is a part-time fixed-term position, working 4 days a week until 30 June 2025. The role is based in a hybrid work environment in Perth, with some regional travel required.

Key Responsibilities
  • Deliver individual and group information and education sessions to people living with dementia, their families, and carers.
  • Provide specialist assistance to help individuals understand and access aged care services and connect with community supports.
  • Ensure services are delivered from an enablement perspective, empowering clients to take an active role in directing their care and services.
  • Build relationships with regional stakeholders and leverage internal and external resources to develop responses to consumer needs.
  • Develop and embed referral pathways to ensure individuals are referred to the most appropriate service for their needs.
  • Some regional travel is involved, with occasional overnight stays.

About You

To be successful in this role, you will need:

  1. A formal qualification in a related field, such as Social Work, Allied Health, or Nursing, or significant experience working with clients in a community-based service or health-related field.
  2. Experience and/or knowledge of the impacts of dementia on individuals, carers, and their support networks.
  3. Experience and knowledge of group facilitation and group dynamics.
  4. A commitment to quality, planning, and efficient resourcing.
  5. Excellent communication and interpersonal skills.
  6. A strong ability to build and sustain relationships, both internally and externally.
  7. A commitment to ongoing professional development and the application of specialist and technical skills to achieve work outcomes.
