Home Health Therapist Role

We are seeking a skilled Physical Therapist to join our team.

  • Evaluate patient functional status and physical therapy needs, developing treatment plans to improve well-being.
  • Provide services within the scope of practice, consulting with physicians and healthcare teams as needed.
  • Treat patients to relieve pain and restore or maintain function, instructing them on self-care and progress.
  • Visit patients in their homes, completing progress notes and submitting reports according to policy.
About the Job
  • Assess and evaluate patient care needs related to mobility, endurance, pain, and medical conditions.
  • Develop treatment plans and perform interventions to improve patient well-being.
  • Evaluate patient response to treatment and report changes to the nurse supervisor and physician.
  • Instruct and inform healthcare team members, patients, and families about the plan of care and progress.
Requirements and Qualifications
  • Licensure Requirements: Current Physical Therapy licensure in the state of practice, Current CPR certification, Valid driver's license, vehicle insurance, and access to reliable transportation.
