Job Description:

We are seeking a compassionate and skilled Occupational Therapist to join our team in providing high-quality home health care services.


About the Role:
  • Evaluate patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
  • Determine treatment plans, perform interventions, and evaluate patient progress.
Responsibilities:
  1. Provide occupational therapy services within the scope of practice as defined by state laws governing Occupational Therapy.
  2. Consult with physicians and participate in multidisciplinary teams.
  3. Instruct patients, family members, and healthcare teams on treatment plans and progress.
  4. Complete patient evaluations, develop OT plans of care, and report outcomes to nurse supervisors.
  5. Participate in staff conferences, committees, and continuing education to maintain clinical knowledge and licensure.
