Clinical Care Coordinator Position Overview

This is a senior management role in the aged care sector, where you will lead clinical and care teams to provide exceptional care to residents. As a Clinical Care Coordinator, you will play a pivotal role in our home's clinical governance and may manage specialized clinical portfolios.

The ideal candidate will be a registered nurse with senior management experience, in-depth knowledge of the aged care industry, and strong negotiation skills. They should also be proficient in utilizing various information technology tools and software.

  • Key Responsibilities:
  • Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff.
  • Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
  • Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
  • Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
  • Conduct routine clinical assessments to promptly address identified gaps in care.
