Dynamic Aged Care Leadership Role

As a leader in the aged care sector, this role offers a unique opportunity to make a meaningful difference in the lives of seniors. The Team Leader will be responsible for overseeing a team that delivers essential support services through the Commonwealth Home Support Programme (CHSP).
About the Role- Service Delivery and Coordination: The successful candidate will ensure that service delivery aligns with contracts and meets the needs of clients.
- Quality Standards and Reforms: Implement processes and procedures to comply with reforms and aged care quality standards, ensuring that the highest level of care is maintained at all times.
- Clients and Community Engagement: Increase awareness and engagement among seniors, promoting Choices at Home as a trusted provider of home-based support services.
- Partnerships and Collaborations: Foster strong relationships with local NGOs, health professionals, and community groups, working together to deliver high-quality services.
- Team Management and Processes: Coordinate client service agreements, onboarding, assessments, referrals, and addressing service gaps, ensuring seamless delivery of services.
