Professional Working With Elderly Individuals

Our organisation has implemented the Single Aged Care Assessment System (SAS) to streamline access to aged care services. This system encompasses assessments for various aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments.
In this role, you will be part of the SAS team, reporting directly to the Clinical Team Leader. Your primary responsibilities include:
- Conducting client assessments, reviews, and developing support plans to facilitate clients' goals and independent living in their homes and community.
- Participating in clinical triage and case conferencing activities.
- Identifying, monitoring, and managing risks, with appropriate escalation as needed.
Key Requirements and Qualifications:
- Demonstrated experience working in Australia or overseas in aged care settings, with direct delivery of services to elderly individuals.
- Tertiary qualification in a health-related field directly related to health, aged care, or a specialist area such as Allied Health or Social Work.
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or relevant professional association.
- Enrolled Nurses with at least one year's experience in Aged Care Needs Assessment.
