Postgraduate Aftercare Manager

Job Description:

The postgraduate aftercare manager will oversee the delivery of temporary full-time aftercare services in a district-wide setting, integrating aftercare services with hospital and community mental health and suicide prevention teams. The role requires coordination with senior health managers, clinical leaders, research partners, and other external stakeholders to enhance service visibility for patients/consumers, families, and clinicians.

Key Responsibilities:
  • Develop and implement strategies to improve referral pathways and service delivery in collaboration with district teams.
  • Provide leadership in clinical and operational governance consistent with NSW Health policies, procedures, and standards.
  • Lead complex multi-site project management and change management initiatives in a health setting.
Required Skills and Qualifications:
  • Tertiary qualifications or equivalent work experience in mental health and suicide prevention.
  • Proven experience in project management and change management in a health setting.
  • Demonstrated ability to work collaboratively with senior health managers and clinical leaders.
