About the Role

The successful candidate will be responsible for assessing and treating patients, developing and implementing individualised care plans, and collaborating with other members of the healthcare team to ensure seamless care delivery.

About You

  • You must hold tertiary qualifications in Social Work.
  • You should possess highly developed assessment and treatment skills.
  • Eligibility for membership with the Australian Association of Social Workers (AASW) is essential.
  • You should be able to work individually and in a team environment.
  • Demonstrated ability to manage a clinical caseload is required.
  • You must understand the role of a Social Worker within a multidisciplinary team.
  • Strong collaborative interpersonal skills with well-developed verbal and written communication skills are necessary.
  • Immunisation evidence and serology report in accordance with NSW Health are required.
  • Experience of complex discharge planning and working within a hospital inpatient environment would be advantageous.
