The Community Rehabilitation Program at Eastern Health is seeking a skilled Physiotherapist to join our dynamic team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality rehabilitation services to clients in a community setting.

Key Responsibilities
  • Develop and implement individualized rehabilitation plans to optimize client functional abilities and independence
  • Conduct thorough assessments and evaluations to identify client needs and goals
  • Collaborate with multidisciplinary team members to ensure comprehensive care and support
  • Provide education and training to clients and their families on rehabilitation techniques and strategies
  • Participate in quality improvement initiatives and contribute to the development of best practices in community rehabilitation
Requirements
  • Recognized tertiary qualification in Physiotherapy
  • Current registration with the Physiotherapy Board of Australia
  • Eligibility for practicing membership of the Australian Physiotherapy Association
  • Advanced clinical skills in managing a complex caseload and comprehensive discharge planning using a client-centered approach
  • Broad clinical experience encompassing diverse patient presentations
  • Previous clinical experience working in an aligned clinical environment
  • High-level communication skills, both written and oral
  • Demonstrated capacity to work/lead in a multidisciplinary team
  • Experience in supervision of staff and/or students
  • Demonstrated ability to introduce innovative practice
  • Commitment to improving own practice and undertaking quality and research activities
  • Demonstrated understanding of professional codes of conduct
