Role Description:

Delivering Personalised Care and Support

As a Clinical Care Partner, you will work with older clients who may have complex life experiences. Your role will involve co-designing care plans that are safe, effective, and tailored to each client's needs.

  • You will conduct clinical assessments to develop care plans that support the client's goals.
  • Establish care and service plans in line with funding guidelines and the client's package parameters.
  • Make referrals to allied health professionals and service providers to ensure agreed care plans are fully delivered.

The ideal candidate will have:

  • AHPRA registration as a Registered Nurse within the aged care or health sector.
  • Excellent people skills, enabling you to build strong relationships and work collaboratively with multidisciplinary teams.
  • Proven experience in delivering person-centered care and support, with a focus on promoting client wellbeing.
  • Deep understanding of the challenges, needs, and goals of older people, their families, and carers.
  • Experience working with diverse and vulnerable populations.
  • Knowledge of aged care funding systems and Home Care Packages (HCP).
