We are seeking a dedicated and passionate Case Manager to join our team in Northern NSW. As a Case Manager, you will work closely with children, young people, their families and carers to create and implement case plans that strive towards achieving the permanency goals of those we support.
The ideal candidate will have a relevant bachelor's degree such as Social Work, Social Welfare, Psychology or similar, and previous case management experience within an Out of Home Care (OOHC) /Child Protection setting. Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect is also essential.
- Building and maintaining strong, functional relationships with both the children and their families, ensuring that every voice is heard and every need is met.
- Monitoring placements, guiding carers in their foster care journey and helping them implement effective strategies that best support the children and young people within their care.
