We are seeking a skilled professional to fill the role of Clinical Care Coordinator. This pivotal position requires strong clinical governance, oversight, and operational management across both NDIS and Home Care Packages.
Key Responsibilities:- Provide leadership and guidance to ensure quality, compliant services.
- Oversee complex care plans, medication administration, and clinical interventions (including wound care, catheter care, stoma care, palliative care, and more).
- Deliver training and coaching to support Home Care Workers in their roles.
- Manage incident and risk processes to ensure timely resolution.
- Assess and support customers living with dementia or cognitive impairment.
- Conduct falls risk assessments and implement prevention strategies.
- Collaborate with healthcare professionals, clients, and families to achieve best-practice outcomes.
- Current registration as a Registered Nurse through AHPRA.
- Evidence of continued professional development.
- Minimum 3 years' experience in aged care or community care.
- Proven leadership skills with the ability to mentor and develop staff.
- Strong knowledge of Aged Care Quality Standards and relevant legislation.
- Experience in complex clinical assessments and care planning.
- Proficiency in Microsoft Office (Word, Excel, Outlook).
