Care Manager – Aged Care | Tweed Heads Region

Location: Tweed Heads Region, NSW

Schedule: Monday - Friday

Role Type: Permanent, Full-Time

Are you a compassionate and experienced nurse ready to make a meaningful impact in aged care? We're seeking a Care Manager to join our dedicated team in the beautiful Tweed Heads region.

About the Role

As the Care Manager, you will work closely with the General Manager (GM) to ensure the highest standards of care and compliance across the home. You will provide clinical leadership, oversee care planning, and ensure resident wellbeing, while also driving quality improvements and regulatory compliance. You'll act as a mentor to staff, fostering a culture of safety, professionalism, and compassion.

Responsibilities
  • Lead the delivery of high-quality, person-centred care in line with the Aged Care Quality Standards.
  • Work collaboratively with the GM to manage compliance, audits, and accreditation requirements.
  • Oversee resident assessments, care plans, and clinical documentation to ensure accuracy and compliance.
  • Mentor, coach, and support nursing and care staff, building capability and confidence.
  • Monitor and improve medication management processes, clinical governance, and risk management.
  • Partner with residents and families to ensure transparent communication and strong relationships.
  • Contribute to workforce planning, rostering, and staff performance management.
  • Drive continuous improvement initiatives to enhance resident outcomes and service delivery.

About You
  • Current AHPRA registration as a Registered Nurse
  • Extensive experience in aged care with strong clinical and leadership skills
  • Proven ability to manage compliance, quality systems, and audits
  • Skilled in care planning, documentation, and clinical governance
  • Strong communication and interpersonal skills with residents, families, and staff
  • Passionate about leading teams and delivering excellence in aged care
