Job Title: Disability Support Specialist

As a Disability Support Specialist, you will play a vital role in empowering individuals with disabilities to achieve their goals and live independently.

This role involves working closely with clients to understand their needs and preferences, and implementing support plans that cater to their requirements. You will also be responsible for coordinating services, managing resources, and advocating for the rights of clients.

To excel in this position, you should possess excellent communication and interpersonal skills, as well as strong problem-solving abilities. Relevant qualifications in social work or community services are highly desirable, and experience in case management or support coordination is a plus.

Key Responsibilities:

  • Client Support: Build strong relationships with clients and understand their goals, preferences, and needs.
  • Plan Implementation: Assist clients in implementing their support plans, ensuring they access necessary services and resources.
  • Service Coordination: Liaise with service providers to coordinate and facilitate the delivery of services, ensuring timely and effective support.
  • Resource Management: Help clients manage their funding, including budget tracking and resource allocation.
  • Advocacy: Advocate for the rights and needs of clients, ensuring their voices are heard in all aspects of their support.
  • Monitoring Progress: Regularly review and assess the effectiveness of supports, adjusting plans as necessary to meet changing needs.

Required Skills and Qualifications:

  • Relevant Qualification: Bachelor's degree in social work, community services, or related field.
  • Knowledge and Skills: Strong understanding of the disability sector, excellent communication and interpersonal skills, and ability to work independently and as part of a team.
