MannaCare is seeking an experienced and compassionate Admissions/Intake Coordinator to join our Community Team. This role offers the opportunity to make a meaningful difference in the lives of older adults, providing exceptional care and support services.

About the Role

We are currently looking for a dedicated individual to manage the smooth administration of new client intake for our Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP). Key responsibilities include:

  • New Referral Management: Review the My Care Portal for new referrals and coordinate appointments for client admission into the CHSP program.
  • Quality Customer Service: Provide exceptional customer service to potential new clients, ensuring their needs are met efficiently and effectively.
  • Client Information Management: Enter all client information into the client management system with precision and attention to detail.
  • Initial Assessment and Planning: Complete initial assessments and develop service/care plans that meet the unique needs of each client.
  • Database Record Keeping: Maintain accurate and up-to-date database records, following up with clients and case managers as necessary.
  • Collaboration and Communication: Work closely with internal departments to respond to client referrals and inquiries, fostering positive stakeholder relationships and engagement.
About You

To be successful in this role, you will require:

  • Aged Care Experience: Proven experience working in an aged care environment, with a strong understanding of the CHSP and HCP funding streams.
  • Nursing Qualifications: Relevant nursing qualifications with AHPRA registration (EN/RN).
  • Sales Understanding: Demonstrated understanding of sales principles and techniques.
  • Data Entry and Referral Management: Experience with data entry, referral management, portals, and other referral media.
  • Microsoft Office Skills: Strong Microsoft Office skills, with the ability to learn and adapt to various client management systems.
  • Communication and Time Management: Excellent communication skills, with the ability to manage time effectively and prioritize tasks.
  • Self-Motivation and Teamwork: Self-motivated, with the ability to work independently and as part of a team.
  • NDIS Working Screening Check and Driver's License: Current NDIS Working Screening Check and a valid driver's license, comprehensively insured and registered vehicle.
