Job Title: Supporting Young Lives Through Case Management

We are seeking a dedicated and passionate individual to join our team as a Case Manager for the Fostering Young Lives program.

This role involves working with children and young people in Out Of Home Care (OOHC), supporting them, their carers, and birth families to achieve the best possible outcomes.

The successful candidate will have excellent communication skills, be able to work holistically, and ensure the home and support network contribute to the development of the child/young person.

They will also be able to adapt easily to change and prioritize the well-being of the children at all times.

Key responsibilities include:

  • Working with a range of designated clients
  • Engaging with clients, including children, young people, and their families
  • Conducting thorough assessments to develop, implement, and review client plans
  • Facilitating transparent communication by openly sharing client plans with clients and relevant agencies
  • Coordinating with various service providers to ensure comprehensive support delivery
