Clinical Care Partner

As a skilled and compassionate healthcare professional, you will play a pivotal role in delivering high-quality care and services that meet the individual needs of older adults.

The successful candidate will possess excellent assessment and planning skills, with the ability to conduct thorough assessments to determine needs and identify clinical risks. They will also be responsible for developing effective care plans and budgets to meet the needs and address clinical risks.

Key Responsibilities:

  • Support intake and leadership functions when high need/high risk care recipients require clinical input/consultation.
  • Conduct comprehensive assessments to determine needs and identify clinical risks.
  • Develop personalized care plans and quarterly budgets to meet needs and address clinical risks.
  • Ongoing management of risks to participants' health, safety, and wellbeing.
  • Collaborate in the development of Support at Home procedures and work instructions.
  • Assist in maintaining the After-Hours roster.
  • Share responsibility as one of the After-Hours Service contacts.

Requirements:

  • Bachelor of Nursing or equivalent qualification.
  • Current unrestricted registration with a relevant professional association.
  • Experience in case management, participant assessment, and care planning.
  • Sound understanding of Support at Home and the Strengthened Aged Care Quality Standards.
  • Demonstrated ability to provide person-centred, holistic services.
  • Financial skills to effectively manage package expenditure for participants within a framework of participant-directed care and choice.
  • Highly developed written and oral communication skills, interpersonal, and organisational skills.
  • Demonstrated ability to work autonomously and as a member of an effective team.
  • Knowledge of My Aged Care portal and Participant Information Management Systems.
