Clinical Aged Care Professional
About the Role:We seek a skilled and empathetic Clinical Aged Care Professional to work with our customers in Hervey Bay.
The successful candidate will conduct comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate and determine customer needs.
The role involves developing personalized support plans, assisting with linking support, reablement, and short-term case management, and participating in clinical supervision.
Key Responsibilities:
- Collaborate and communicate with customers, carers, families, representatives, and other service providers during the aged care planning process.
- Undertake and review personalized support plans, managing changing customer and carer needs with their consent.
- Identify customers' formal and/or informal respite support and arrangements.
- Develop an Emergency Care Plan if the customer does not have one in place.
- Refer for services, assess complexity, and risk of vulnerability, and assist with linking support, reablement, and short-term case management.
Applicants must hold a tertiary qualification in a health-related discipline, possess current unrestricted registration with AHPRA, and have at least one year of experience delivering services in aged care settings.
The ideal candidate will possess exceptional customer service skills, knowledge or experience conducting home assessments or risk assessments for vulnerable people, and adaptability to respond to changing task requirements.
