We are seeking a skilled and experienced mental health professional to join our multidisciplinary team in Gladstone, Queensland.
As a Senior Care Coordinator, you will be responsible for providing comprehensive case management services to adult clients in the region.
- Key responsibilities include assessing client needs, developing individualized care plans, and coordinating with other healthcare professionals to ensure seamless care delivery.
- Proven experience in mental health case management, sound knowledge of adult mental health, and current registration as a Registered Nurse are essential qualifications for this role.
- Relevant post-graduate qualifications or equivalent experience in mental health nursing are highly desirable.
