Job Description:

We are seeking a highly skilled and experienced mental health professional to join our team as a Case Manager. In this role, you will be responsible for conducting triage and assessment of client mental health referrals, conducting urgent risk assessments, claims management, short-term case management intervention, and providing an escalation point for clients at risk.

Key Responsibilities:

  • Conduct triage and assessment of client mental health referrals
  • Conduct urgent risk assessments
  • Claims management
  • Short-term case management intervention to link clients to necessary services for ongoing support
  • Provide an escalation point for clients at risk to harm
  • Manage a caseload of clients
  • Undertake a range of assessments including intake assessments and risk assessments
  • Preparation of high-level client summaries and briefs including maintaining high-level documentation

Required Skills and Qualifications:

To be successful in this role, you will need to have relevant tertiary qualifications within Nursing, Psychology, Occupational Therapy or Social Work. You will also need to have current registration with the Australian Health Practitioners Registration Agency (AHPRA) or Australian Association of Social Workers (AASW). Additionally, you will require experience in a similar clinical or case management environment, and a passion for clinical psychology, social work or mental health nursing to support mental health initiatives.

Benefits:

This is a fantastic opportunity for the right candidate to join our team on a full-time basis, Monday to Friday, in a Brisbane CBD location. The hourly rate is $60 + super, and you will have access to ASAP start (pending Federal Police Check) and a 1-2 year temporary assignment.
