Your Key Responsibilities
  • Provide comprehensive support to individuals with dementia through home visits and clinic-based services
  • Work collaboratively with other healthcare professionals to deliver effective care plans
  • Develop and implement individualised care plans that promote independence and quality of life
  • Conduct thorough assessments and evaluations to identify patient needs and develop appropriate interventions

  • You are a compassionate and proactive clinician who connects easily with patients and families
  • You possess strong time management, assessment, and communication skills
  • You bring a genuine interest in dementia care and an eagerness to support carers and community teams
  • You have the cultural awareness and empathy to support diverse populations
Requirements
  • Current registration with AHPRA as a Registered Nurse
  • Proven clinical experience in dementia, delirium, or aged care case management
