As a Support Clinician, you will play a key part in delivering high-quality care services for individuals with dementia and aged care needs.
This role is ideal for a compassionate and self-motivated professional who thrives in a person- and family-centred care environment and enjoys working both independently and within a collaborative, multidisciplinary team.
Your Key Responsibilities- Provide comprehensive support to individuals with dementia through home visits and clinic-based services
- Work collaboratively with other healthcare professionals to deliver effective care plans
- Develop and implement individualised care plans that promote independence and quality of life
- Conduct thorough assessments and evaluations to identify patient needs and develop appropriate interventions
- You are a compassionate and proactive clinician who connects easily with patients and families
- You possess strong time management, assessment, and communication skills
- You bring a genuine interest in dementia care and an eagerness to support carers and community teams
- You have the cultural awareness and empathy to support diverse populations
- Current registration with AHPRA as a Registered Nurse
- Proven clinical experience in dementia, delirium, or aged care case management
