Support Coordinator Job Description

We are seeking a skilled Support Coordinator to link NDIS participants with services, supports and programs according to their individual needs and goals.

About the Role:
  • To develop and maintain effective collaborative relationships with NDIS participants, families, carers, advocates, and other service providers.
  • To review and monitor NDIS plan outcomes and expenditure in conjunction with plan managers to ensure the plan remains in budget.

Key Responsibilities:

  1. To oversee support coordination in accordance with participants' NDIS plans.
  2. To work collaboratively with a strong alignment to our core values.

Requirements:

  • A minimum of 12 months experience in a Support Coordination role, LAC, NDIS planner or similar.
  • Experience working with people with a disability living within complex situations.
  • Demonstrated experience in interpreting and implementing individual NDIS plans.
