Main Responsibilities- Undertake comprehensive assessments of patients to inform individualized care plans.
- Collaborate with multidisciplinary teams to identify and implement discharge planning requirements.
- Develop and prioritize care plans to achieve optimal patient outcomes.
- Evaluate patient progress and revise nursing interventions as necessary.
