Job Overview

We are seeking a compassionate and skilled Support Worker to join our dedicated team. As a Support Worker, you will play a vital role in providing exceptional care and companionship to our clients.

Your primary focus will be on delivering high-quality care that meets the individual needs of our clients. You will work closely with our Care Designers to ensure seamless continuity of care and excellent client outcomes.

Main Responsibilities
  • Provide professional companionship and personal care to clients based on their individual needs and an established/documented care plan.
  • Work directly with the client, their families and friends, reporting to their designated Care Designer ensuring continuity of care.

Key Skills and Qualifications

  • Certification in Aged/Disability/Community Care (or related field).
  • Two positive professional references.
  • National Police Check (Aged care specific).
  • Working with Children Check.
  • Ability to travel independently and reliably to client visits.
  • Excellent communication skills.
