Lead Patient Support Specialist

We are seeking a skilled Social Worker to provide high-quality social work services to patients and their families in a dynamic multidisciplinary team environment.
- The successful candidate will be responsible for providing effective and efficient support and advice to patients, their families, and carers;
- Participating as a member of the multi-disciplinary team in assessing patients' level of functional and care needs, setting care goals and providing appropriate treatment plans;
- Providing support and guidance to patients and their families in navigating the healthcare system;
- Assessing patients' level of functional and care needs, identifying areas for improvement, and developing personalized care plans;
To be considered for this role, you will need to demonstrate:
- Commitment to providing high-quality patient-centered care;
- Tertiary qualification in Social Work or equivalent;
eligibility for membership with the Australian Association of Social Workers (AASW); - Experience working in Rehabilitation, Older Persons Mental Health, and Palliative Care environments;
- Strong communication and interpersonal skills, with the ability to interact effectively with patients, families, and internal/external service providers;
- A commitment to ongoing professional development, learning opportunities, and supervision;
- An unencumbered Driver's License.
