The Opportunity

We are pleased to share this opportunity for a leadership role in a dementia care initiative, supporting individuals with dementia transitioning from hospital settings back to their homes or into residential care.

The Role

The Team Leader ensures that the Dementia Support Australia service is tailored to individual client needs and meets its key performance indicators.

This role is a temporary full-time or secondment opportunity until 2028, supporting our team in Tasmania.

Responsibilities
  1. Provide strategic leadership to a multidisciplinary team across up to three locations (Hobart, Launceston, and referrals from North West Tasmania), focusing on the DSA Program in line with policies, procedures, and organisational values.
  2. Collaborate with an integrated team to ensure a consistent approach to our 'Next Chapter' initiative.
  3. Develop a culture of empowerment, mentorship, and support for the team, ensuring wellbeing and safety.
  4. Oversee service delivery to operate within the case management framework, reducing the impact of behaviours on people with dementia, their carers, and staff.
  5. Contribute to program objectives related to administration, finance, risk, quality, and reporting.
  6. Provide direct support and consultation to clients as needed.
  7. Ensure timely referral allocation to DSA staff aligned with KPIs.
  8. Report regularly to the Regional Manager on staff performance, service quality, and operational issues.
  9. Recruit suitable Dementia Consultants, Support Coaches, and Family Liaison Officers.
  10. Manage stakeholder relationships with local managers and staff in aged care, home care, and hospitals to promote program understanding and effectiveness.
  11. Handle customer complaints or challenging situations promptly, notifying management when necessary.
  12. Address incidents and feedback related to service quality with the Regional Manager.
  13. Enhance personal knowledge and skills through ongoing learning.
  14. Ensure a safe office environment, including maintenance and equipment management.
  15. Oversee the use and maintenance of fleet vehicles, including tracking and reporting.
