Support Facilitator

We are seeking a dedicated and compassionate Support Facilitator to join our team at Relationships Australia South Australia. As a Support Facilitator, you will play a vital role in assisting young people who are transitioning to independence after experiencing the child protection system.
Key Responsibilities- Develop and implement individual client objectives to promote independent living skills, including financial literacy, housing, and social connections.
- Conduct holistic screening and risk assessment using the DOORS tool and other assessments to identify client needs and develop effective support plans.
- Provide trauma-informed, collaborative case management and support coordination services through face-to-face, outreach, telehealth, and phone to ensure clients receive comprehensive support.
- Facilitate client-led decision-making processes to empower clients to make informed choices about their lives.
- Collaborate with the Senior Support Facilitator to develop and facilitate group activities that promote social connections and community engagement.
- Bachelor's Degree (AQF Level 7) in Social Work or demonstrated knowledge and competencies appropriate to Social Work.
- Experience in delivering trauma-informed practice and working with Aboriginal and Torres Strait Islander young people, families, or communities effectively.
- Strong communication and interpersonal skills to build rapport with clients and colleagues.
