Job Summary:

This is an ongoing announcement for a Nurse Practitioner to provide comprehensive and compassionate care for patients. The ideal candidate will have a high level of medical knowledge, excellent communication skills, and the ability to lead a team.

  • Key Responsibilities:
  1. Deliver primary care services to a defined patient panel, including history taking, physical examination, ordering laboratories and diagnostic studies, diagnosis and formulation of treatment plans for acute and chronic conditions within the scope of Medicine practice.
  2. Lead a Patient Aligned Care Team (PACT), collaborating with healthcare providers to deliver high-quality patient-centered care.
  3. Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Qualifications:
  1. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies:
    The Accreditation Commission for Education in Nursing (ACEN) or
    The Commission on Collegiate Nursing Education (CCNE).
  2. APNs must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
  3. All APNs must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
  • Requirements:
  1. Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
