Mosaic Multicultural Connections is a not-for-profit organisation providing settlement, aged care, youth, and vulnerable families services across NSW regions. We are seeking a permanent Care Manager to coordinate Home Care and Commonwealth Home Support Respite packages for clients from Culturally and Linguistically Diverse (CALD) backgrounds.

About the Role

The successful candidate will be responsible for case managing and coordinating the delivery of services, ensuring they comply with Aged Care Quality Standards. Key responsibilities include:

  • Conducting in-home assessments to identify client needs,
  • Developing care plans in consultation with clients and their carers,
  • Coordinating service implementation to meet client goals within their program,
  • Ensuring services meet quality standards.

Requirements

To be successful in this role, you will need:

  • A Certificate III or IV in Individual Support or equivalent qualification/experience,
  • Experience in a similar Coordinator role within the industry,
  • High level organisational skills, ability to meet diverse client needs,
  • Experience working with CALD clients (desirable),
  • Proficiency in Microsoft Office suite,
  • Willingness to participate in after-hours on-call services.
