As a Client Service Coordinator, you will play a vital role in ensuring older Australians receive exceptional care and support to live independently.

About the Role

This position requires strong interpersonal skills to build and maintain relationships with clients and their families, ensuring they receive the best possible service. You will oversee all aspects of service delivery, working closely with internal teams to meet each client's unique needs.

Key Responsibilities

  • Contact clients and their families to provide accurate information about available services.
  • Maintain client feedback records, addressing concerns in line with relevant policies and procedures.
  • Conduct regular wellness visits, client contacts, and annual reviews to optimize care and support.
  • Develop care plans aligned with assessments and client goals, collaborating with clinical and allied health teams.
  • Review and update care plans annually or as care needs change.
  • Manage client resources effectively to prevent deficits or surpluses.
  • Maintain accurate budget planners, updating them as services or funding levels change.
  • Visit clients in person as required.

About You

You are a compassionate and customer-focused professional who excels in a dynamic environment. Your ability to deliver exceptional service and coordinate resources effectively will ensure clients receive the best possible support.

  • Evidence of COVID-19 vaccination.
  • A National Police Clearance (valid for 6 months).
  • Successful completion of pre-employment health checks and reference checks.
  • A valid driver's licence.
  • A reliable vehicle with comprehensive insurance.
  • Desirable: A Certificate IV in Community Service Coordination or Enrolled Nurse qualification.
  • A willingness to complete a paid induction at our training institute.
