Job Summary:

The Clinical Care Coordinator role involves providing clinical guidance and collaborating with healthcare professionals to ensure residents receive high-quality care.

Key Responsibilities:
  • Clinical decision-making and supervision of care for individual residents, including wound care, pain management, and end-of-life care.
  • Liaison with healthcare staff, medical practitioners, and allied health specialists to facilitate effective care coordination.
Requirements:
  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
  • Recent experience in caring for high-care and dementia residents, with a focus on complex care needs.
  • Expertise in wound care, palliative care, behaviour management, and clinical assessments.
