Job Overview

Care Facilitators play a vital role in empowering clients to live independently in their homes.

About the Role

The Care Facilitator will assist clients and their families by engaging them in services, enabling them to remain living in their home. This involves developing individualised care plans and setting realistic goals that cater to each client's needs.

Key Responsibilities
  • Conduct home visits to assess client needs and provide support as required.
  • Collaborate with multidisciplinary teams to develop and implement effective services and programs that meet client needs.
  • Establish strong relationships with families and carers, acknowledging their requirements and providing tailored support.
  • Work closely with clients to set achievable goals, promoting their independence and well-being.
  • Implement best practice approaches, ensuring evidence-based care delivery.
  • Maintain accurate records using client management systems.
Requirements
  • Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management) or equivalent knowledge and experience.
  • Experience working in aged care or transferable skills highly regarded.
  • Demonstrated ability to operate within funding constraints and achieve service delivery objectives within government program guidelines.
  • A valid NSW driver's licence.
  • Intermediate level of computer skills.
  • Excellent time management and organisational skills.
  • Ability to work independently and as part of a team.
