**Home Care Support Role**

We are seeking a skilled and empathetic individual to join our team as a Home Care Support Worker. In this role, you will have the opportunity to make a meaningful difference in the lives of older Australians and those facing disadvantage.

**Job Description:**

  • You will visit clients in their homes, providing support and care to enable them to live independently.
  • You will work closely with clients and their families to develop individualised care plans that meet their unique needs.
  • You will participate in the development and implementation of services and programs that promote client well-being.

**Key Requirements:**

  • A bachelor's degree or diploma in community services (service coordination or case management)
  • Experience working in aged care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
