Diabetes Education Role Overview

We are seeking an experienced and dynamic educator to join our primary health team on a fixed term part-time basis. The successful candidate will be a registered nurse or accredited practising dietitian with credentialed diabetes educator (CDE) status or working towards the same.

The role involves providing diabetes education and support to patients, collaborating with multidisciplinary team members, developing and implementing individualised care plans, and liaising with other healthcare professionals to ensure seamless care.

Required Skills and Qualifications:

  • Advanced clinical skills in acute and/or community nursing or dietetics
  • Experience in providing diabetes education and support
  • Credentialed Diabetes Educator (CDE) status or working towards the same
