Role Overview:
This part-time position is designed to provide comprehensive client assessments for older people across the Loddon Mallee region in Victoria, Australia.
The successful candidate will work alongside multidisciplinary teams to provide clinical support and develop comprehensive care plans.
Main Responsibilities:
- Conduct face-to-face, telehealth, or phone assessments for older people
- Work collaboratively with community-based services to ensure seamless care transitions
- Develop and implement effective care plans that meet individual needs
- Participate in professional development opportunities to enhance skills and knowledge
