Advanced Mental Health Care Coordinator

We are seeking an experienced Advanced Mental Health Care Coordinator to join our dynamic multi-disciplinary community team.
Key Responsibilities:- Provide education and guidance to internal and external partners, including Residential Aged Care Facility staff on advanced mental health care practices.
- Conduct comprehensive mental health assessments and develop, implement, and evaluate consumer-focused care plans based on evidence-based practice.
- Assess, plan, implement, and evaluate interventions and care plans for consumers presenting with complex mental health needs and/or behaviors of concern.
- Registered Nurse with a minimum of 5 years full-time post-graduate experience in a mental health specialty.
- Approved post-graduate qualification within the speciality of mental health drug and alcohol or other such qualifications or experience deemed appropriate.
- Demonstrated experience working with consumers and their carers presenting with complex mental health needs and/or behaviors of concern.
